The program for cervix cancer now follow up in Phnom Penh, Takmao
Kompong Cham and Battambang
•
Since 6 years we are developing a program called” cervix
cancer early detection , diagnosis and treatments“
• This is the first prevention action in the area of cancerology
in the country ;
• Up to date 8000 women have benefited of detection, 800 of biopsy , 460 have a cancer diagnosis and 300 a curative treatment ( 40% by simple surgery in according with early diagnosis )
• All the patients whatever are their incomes can benefit
the program
• The ministry of health of the kingdom of Cambodia request
us to extend the program in provincials areas,
in Kompong Cham and Battambang
This program is supported by the International union against
cancer and the French coalition against cancer and the mission
for non governmental co-operation of the French ministry of
foreign affairs

• The second is "Global program for diabetic people"
2 years ago we implemented the department of diabetology in Preah Kossamak Hospital :
The results for 1° semester 2006 are :
The project intended to reach 1.500 patients every year.
During the first semester more than 1.200 has been reached. So the prognostic is to reach about 3.000 patients at the end of the first year. It is not necessary to modify the design.
The appropriateness is efficient.
The assessment in the diabetology department of Kossamak hospital shows:
- Quantitatively the number of new patients is increasing
- Qualitatively the average HbA1C has decreased between January and June
- January : average HbA1c = 8.8% , 31% of patients exceeded 10%
- June : average HbA1c = 6.5% , 23% of patients exceed 10%

The number of doctors who have benefited from medical training is 13 as foreseen.
The number of paramedical staff and educators who have benefited from training is 8.
The number of people who have benefited from health education is about 1.500
The areas for information are the diabetology department of Kossamak Hospital and the MoPoTsyo office.
The number of patients for first consultation is 405 and for follow up is 1.454; about 70% return and continue to come.
At the end of the first quarter of the project:
- The number of patients with good balance was 15 % at first consultation
- after 4 consultations, the number of patients with good balance is 50 %
- The number of patients with good knowledge was 20 %
- The number of patients with normal BMI was : 70 %
- The number of patients with normal blood pressure was 40 %
- 50 % of the patients are under 50 year old,
- 90 % are under 10 year of diabetes length
- The more frequent complication is neuropathy: about 40 % of the patients
- 25 % of the patients have clinical or biological signs that evocate a beginning nephropathy
- The cardio vascular complications that have been detected clinically are less than 10%
- For the patients who can paid the medicine, the average cost of prescription is 6000 at 8000 riels (1.5 at 2 $). For poor people is free. The very poor people, living in the squats, do not come to hospital.
- The studies about the co-infection hepatitis - diabetes carried out on 200 patients without any selection reveals that:
- 4 % of diabetics have HvB +
- 16 % of diabetics have HvC + ( within ¼ with very high blood glucose > 500 mg/dl)
The studies of Institute Pasteur of Cambodia (2004) carried out on general population mentioned:
- 8 % have HvB +
- 4 % have HvC +

We can confirm the worsening role of the co - morbidity diabetes and hepatitis C that have been described by the Canadian studies.
We know also the worsening role of VIH and tri-therapies that have a great incidence.
All the measures to decrease the viral transmission could be also means to decrease diabetes.
- The studies of glycemic index began in June 2006, because it was necessary to obtain consensus about the protocols and agreement of the ethic comity: in according with the first results the rice washed , the usual Cambodian cooking have very highest index .
